I've played through the main quest about 20 times and I've noticed that on more than a few occasions, Tavia is dead when I first arrive at the Kvatch camp. I always check for enemies but there are never any around, she's always just laying there dead in either a sleeping bag or in the middle of the camp. And its always just her, no one else. Maybe there's some crazy bug that causes her to die or either I just have some messed-up playthroughs.
