[edit] The Gatekeeper
After first entering the Fringe, travel northward along the main path until you reach the small town of Passwall. Speak with Shelden, the self proclaimed mayor of Passwall, to learn of the nature of the Fringe, and more importantly, the Gatekeeper northeast of Passwall.
Traveling further northeast, you will find the Gates of Madness; the entrance to the Shivering Isles proper. The Gates consist of two doors, one leading to Mania, the other to Dementia, both of which are guarded by the Gatekeeper of the Fringe. Already underway is a rather one sided battle between the Gatekeeper and a group of steel plate armored adventurers. The adventurers are no match for the ten foot tall monster. After the adventurers have been killed, their captain will flee the Isles, exiting through the Door to the Heartlands.
A further quest update will suggest speaking to the residents of Passwall about the Gatekeeper. Further questioning of Passwall's denizens will reveal that the keys to the Gates of Madness are sewn into the Gatekeeper's body; to enter the Shivering Isles, one must defeat the Gatekeeper.
[edit] Prepare for Battle
There are multiple methods of defeating the Gatekeeper. The most straight-forward method is simply to fight it as-is. The Gatekeeper is leveled, possesses a large amount of hit points and is capable of healing itself. Fortunately, there are other methods of evening the fight.
[edit] Gatekeeper Arrows
Some residents of Passwall may inform you of Jayred Ice-Veins, a Nord sharpshooter with an obsession to defeat the Gatekeeper. Speak with Jayred, who will reveal that bones taken from a previous Gatekeeper corpse, found at the Gardens of Flesh and Bone, can be fletched into arrows that are particularly effective against the Gatekeeper. He will request your assistance in picking the lock to the Gardens (although you can also use an open spell) and give you a free lockpick for the outer gate. Follow him to the Garden and open the lock however you see fit and defeat the Shambles that guard the courtyard. Jayred will collect the bones, and tell you to return in a few hours for the arrows. When you return, he will have 20 Gatekeeper Arrows for you.
[edit] Relmyna's Tears
Speaking with Nanette Don, with a disposition of 70 or higher, will reveal that the Gatekeeper is especially susceptible to the tears of its creator, Relmyna Verenim. She will further elaborate, explaining that Relmyna visits the creature alone every night at around midnight. Searching Relmyna's room at the Wastrel's Purse will reveal a little more information. There is a love letter to Lord Sheogorath on her desk that explains, amongst other curious things, that her tears seem to burn the creature, agitating the Daedric souls bound to it. Wait until midnight and travel surreptitiously to the Gates of Madness. Take care not to be noticed by either, (as Relmyna will leave and the Gatekeeper will try to kill you) and you will receive a quest update informing you that Relmyna has dropped a handkerchief. Activate the handkerchief and you will be granted three bottles of the poison 'Relmyna's Tears.'
[edit] Madness...? This is Passwall!
Now armed with the appropriate weapons, travel to the Gates of Madness and engage the Gatekeeper. If Jayred is accompanying you, he will attack the Gatekeeper with his own set of Gatekeeper Arrows. If you have acquired both the 'Gatekeeper Arrows' and 'Relmyna's Tears', poisoning the the arrows with the tears is an effective method. After the Gatekeeper has been defeated, take the "Key to Mania" and the "Key to Dementia" from his corpse.
Enter the Shivering Isles through the Gate of your choice. If you choose to enter the door to Mania, you will be granted the Blessing of Mania (Frenzy up to level 12 in 30 feet for 30 seconds). If you choose to enter the door to Dementia, you will be granted the Blessing of Dementia (Demoralize up to level 12 in 30 feet for 10 seconds on target). Travel to Sheogorath's Palace in New Sheoth and speak with him to complete the quest, and receive instructions for the next quest.
- The Gatekeeper will abandon his attack if you enter any buildings or if you stray too far from the Gates. You can use this to gain a brief respite to heal, recharge weapons and restore your fatigue.
- The Gatekeeper can be affected by both the Wabbajack and Mehrunes' Razor. The former may be by design, as Wabbajack is technically a sign of Sheogorath's favor.
- If you want to get free bone arrows, follow Jayred Ice-Veins back to his house after he has collected the Gatekeepers bones and pick up the bone arrows he shoots in his house when he practices. He will shoot around 10 bone arrows each time and then picks them back up. You won't get a bounty for stealing when you pick his bone arrows up.
- Despite their best efforts, it is physically impossible for the band of adventurers to kill the Gatekeeper, as they cannot do any damage.
- Leaving the Shivering Isles before the adventurers fight the Gatekeeper, levelling up and then returning may trigger a bug where the Gatekeeper is not hostile and the adventurers just stand around (if talked to they will give the "I HAVE NO GREETING" response) so the quest does not update. You can get around this by opening the Wait dialog and waiting for an hour, or resting at the Wastrel's Purse. The adventurers will drop dead and the quest will update. Alternatively you can attack and kill the adventurers yourself, this will cause the their leader to flee and the game to move on. PC users in this state who wish to see the battle can do the following: open the command console, click on the Orc adventurer captain, and type the following exactly:
set GatekeeperRef to 0
set startFightDoOnce to 0
set fightStage to 0
- Close the console and the battle should proceed normally, although the captain won't join in.

- You gain the Blessing of Mania/Dementia before the Autosave for opening the door takes place. Thus, you can gain the Blessing of Dementia, re-load, and go through the Gate of Mania instead, or vice versa.

- Notes
- Not all Journal Entries may appear in your journal; which entries appear and which entries do not depends on the manner in which the quest is done.
- Stages are not always in order of progress. This is usually the case with quests that have multiple possible outcomes or quests where certain tasks may be done in random order.
- If an entry is marked as "Finishes Quest" it means the quest disappears from the Active Quest list, but you may still receive new entries for that quest.
- On the PC, it is possible to use the console to advance through the quest by entering
setstage quest stage. Where quest is the internal name of the quest, and stage is the number of the stage you wish to complete. It is not possible to un-complete (i.e. go back) quest stages. See SetStage for more information.
