Here is a collection of a few of my potion recipes, from back when uh, I - uh, couldn't quite keep it under control. But I'm better now. Honest. I can quit anytime I want!
Except where stated otherwise, these potions are intended for an Expert alchemist. Amateurs beware!
[edit] Cures
It's a good idea to be prepared with cures for ailments you might encounter in your ingredient-hunts.
Note that pure cures always have the same effects (no duration or magnitude) so you can create cures even at low alchemy levels and keep them at full potency throughout the game: do not sell them!
Cure Paralysis
Milk Thistle seeds
Viper's Bugloss leaves
Foxglove nectar
Motherwort sprig -or- Alkanet flower (also gives "light")
Cure paralysis, resist paralysis, resist poison
This is the only Cure Paralysis base pair that does not require rare and potent Imp Gall, and it will prevent whatever paralyzed you from doing it again. The first two are the base pair if you just want a cure
Antivenom (core components)
Ginseng
Strawberry -or- Redwort flower -or- St. Jahn's Wort nectar
Cure poison
A Redwort and Strawberry version can be made at Apprentice level without side effects!
All antivenoms must include Ginseng to keep from being poisonous themselves! However, before Journeyman level you can make it with Redwort flowers and Strawberries, and the strength of this potion never changes, so make as many as you can then. Always a good idea to have some on hand

St. Jahn's Wort nectar
Columbine root pulp
Dryad's Saddle polypore cap -or- Fire salts (omit if you don't need "resist frost")
Cure poison, fortify magicka, chameleon, resist frost
Maintains the versatility of your antivenom ingredients with added effects useful in many situations
Dispel (core components)
Grapes -and/or- Sacred Lotus seeds
Bergamot seeds -or- Mutton
Dispel
The Grapes & Mutton version can be made at Journeyman level
This potion should be made without a Calcinator! It burns off the antimagic essences and perversely weakens the Dispel effect.
A handy potion to have if you're magically silenced for example

Reflect Damage (core components)
Flour
Strawberry -or- Scamp skin -or- Green Stain cup cap -or- Deadra venin
Reflect damage
All reflect damage potions must contain flour as a stabilizer to avoid the "damage health" side effect. Be careful when adding other components.
The ONLY Reflect Spell (core components)
Cinnabar Polypore Yellow cap
Glow dust
Reflect spell
Made of rare ingredients, best used with other ingredients for additional effects, see below...
Countermage

Land Dragon Form
Dragon's Tongue (key ingredient)

Restore health, restore fatigue, resist fire, fire shield, invisibility
Best defense available from fire in a single potion - as everyone knows, land dragons are invisible
Firewalking (core components)
Dragon's Tongue (key ingredient)

Practically any "restore health" ingredient
Restore health, resist fire, fire shield
Best defense available from fire in a single potion from relatively common ingredients
Dragon Armor
Any two of: Lettuce, Crab meat, Cheese wedge
Almost any two of: Beef, Sheperd's pie, Summer Bolete cap, Tomato, Potato, Flax seeds (careful, Tomato with Potato gives "burden" as a side effect)
Fire shield, shield, restore fatigue
A strong shield in a single potion from common food items
Heavy Shield
Potato
Tomato
Morning Glory root pulp
Frost shield, shield, restore fatigue, burden
Add Bread loaf -or- Onion for a "detect life" effect
A strong shield from ingredients common near Skingrad... Burden's not that bad a side-effect is it?
[edit] Poisons
Poisons are more fun and more evil to make than potions. Spread the addiction!
If you have it, be sure to cast weakness to poison before administering any venoms to ensure maximum fast-acting effects.
[edit] Nonlethal poisons
These drugs are meant to disable an enemy without killing them. Note that you'll still need to breach the skin or hide in order to inject the chemicals into the subject's bloodstream, which may sting a bit. Some drugs do leave long-lasting effects but should generally not be fatal.

Add: Wheat grain -or- Ham for "damage magicka"
Damage fatigue, silence
For stunning caster opponents without harming them, made from common ingredients
Sleeping Potion
Lady's Smock leaves
Motherwort sprig
Viper's Bugloss leaves
Monkshood root pulp
Damage fatigue, burden
For stunning opponents without harming them, made from common plants (may need several doses to be effective) This is basically two core pairs thrown together
Ball & Chain
Bread loaf
Tiger Lily nectar
Any two of: Viper's Bugloss leaves, Monkshood root pulp, Fly Amanita cap
Damage strength, burden
For weighing down and immobilizing opponents without harming them, made from common ingredients. (May need several doses to be effective.) Use immobilized enemies as archery targets
Exhaustion
Bread loaf
Tiger Lily nectar
Troll fat, -or-
(Wisp stalk cap -and- garlic)
Damage strength, damage willpower, damage agility
Renders an enemy extremely susceptible to knocking out by fatigue damage (such as sleeping potion) and reduces their damage and ability to stay on their feet - great for cheating in the arena

These venoms reduce an opponent's ability to fight, thereby protecting you.

Health damaging version of the exhaustion poison
