Her Hands' are granted Agility, Attack and Long Blade bonuses via their permanent abilities & equipment, granting them one of the highest hit chances in the game. Vs a player character with Full Fatigue and 100 Agility and 100 Luck (the same base values as one of the hands), Her Hands have a (170 Long Blade + 75/5 or 15 Bonus Agility)*1.25 Fatigue modifier + 50 Fortify Attack = ~281% Hit Rate via the combat formula.

Thus, in order to evade attacks from Her Hands, a character with 100 Base Agility/Luck and full Fatigue requires an additional ~281% Evasion rate. Examples of achieving this include 100 Sanctuary + ~700 bonus Agility (Sanctuary caps at 100 points), or via ~300% Blind (Blind does 'not' cap at 100 points).
